How to Add an Email Form to Your Fyzical.com Website
Create a simple form to collect names, email addresses, phone numbers, questions, comments, or to request information or supply information for your clinic.
An important thing to note is that these forms are not HIPAA compliant - so we want to stay clear of collecting health record information through this form. There are many form builders out there that are HIPAA compliant, just Google "hipaa compliant form builder" and explore your options.
To add an Email form to your website, you can follow our step-by-step guide below:
1) Log in to your FYZICAL Hub Account at hub.fyzical.com/

2) In the main menu click on Content, then head to the Pages submenu.

3) On the left side of the Page editor, under Company Content, navigate to the page you’d like to edit and add a Form to.

4) On the left side of the Page Editor click the + icon that opens up the Elements list.
Then click on Form to see the Form element options.

5) Click and drag the Form element into the Page Editor, and place it where you’d like to add the form. There is a version for light and dark backgrounds.
NOTE: a green line will appear to signal where you’re dropping the new element.

6) As soon as you drop the Form in the page a pop-up window will open where you will need to name the form, and add the contact email address you want to receive the submissions. Then hit Submit.

7) Then, in the Form Fields tab select the fields you would like applicants to fill out - we recommend having name and email address be mandatory requirements, but the rest is up to you.

8) In the Form Settings tab you can adjust additional settings: captcha requirement, how to display field names, link a “Thank You page”, choose the Opt-in settings for further communication with applicants, and choose the follow-up action after submitting a form: Send Email, Campaign, Client Event, None.

9) Once you’re done and you hit Save, you can click away from the Form pop-up and the Form will be added to your page.
If you need to make further changes to the Form at any point, just click on it, and hit the Edit Form button.
